As the story unfolds, it's revealed that English professor Elliot Ward was responsible for the head injury Andie suffered before her death, but it was her younger sister Becca who directly caused her demise.
Becca had been assaulted by Max Hastings and told Andie, but Andie refused to report it, leading to a confrontation between the sisters. Becca pushed Andie, causing her to hit her head and choke, resulting in her death.
The cast, including Emma Myers, was caught off guard by the twist, with Myers revealing she suspected Elliot but not Becca. The TV adaptation differs from the original novel, where Max Hastings is the true culprit.
