Pilots had raised safety concerns at LaGuardia Airport months before the Air Canada crash that killed two pilots and left 41 others hospitalized.

According to CNN, at least one pilot had filed a formal report flagging serious safety issues at LaGuardia Airport through the Aviation Safety Reporting System.

The pilot raised two major issues: air traffic controllers not providing clear guidance on aircraft proximity and a runway lighting system being switched off.

The pilot pleaded with officials to 'please do something' and described a close call where their aircraft was just 300 feet above the ground while approaching runway 22 when another plane was given permission to take off.

On Sunday night, Air Canada Express Flight 8646 collided with a fire truck that had crossed the runway, injuring over 40 people and killing pilots Antoine Forest and Mackenzie Gunther.

Forest, a first officer with Jazz Aviation, was from Coteau-du-Lac, Quebec and had been in the role for just over three years.
